RESPONSIBILITIES
- Demonstrates experience and ability to oversee hospice house functions on his/her assigned shift.
- Provides services requiring substantial and specialized nursing skills, in accordance with the physician orders. Demonstrates competency in all skills required for the agency, including, but not limited to, venipunctures, wound care, infusion therapy, medication administration, infusion therapy device care, and aseptic technique.
- Assesses the patient’s continual care needs and collaborates with Hospice House Manager, RTM, or Patient Care Supervisor and team members regarding patient’s plan of care. Attends and participates in IDT process. Documents nursing assessments, nurses’ notes, medications records, addressing problems in the plan of care or/either rationale for not doing so.
- Follows physicians orders when initiating medical and palliative nursing procedures as appropriate to the patient’s care and safety. Makes referrals to other disciplines as indicated by the patient’s needs or documents rationale for not doing so.
- Observes signs and symptoms and reports to physician and/or other appropriate health professional as often as needed, or upon changes in the patient’s condition.
- Provides services requiring substantial and specialized nursing skills, in accordance with the physician orders. Demonstrates competency in all skills required for the agency, including, but not limited to, venipunctures, wound care, infusion therapy, medication administration, infusion therapy device care, and aseptic technique.
- Teaches, supervises and counsels the patient and family regarding procedures and other care needs as appropriate to the patient’s condition. Utilizes agency educational material as appropriate.
- Supervises the LPN and teaches the nursing assistant where applicable. Assesses the patient’s need for personal care, activities of daily living and revises the plan of care as appropriate. Communicates the plan of care assignments to the assistant and reviews assistants documentation of care as necessary.
- Responsible for following all policies and procedures of hospice and regulatory agencies including, but not limited to, documentation, ordering, storage and record keeping of controlled medications.
- Verbalizes and exhibits knowledge of dealing with patients and families including, but not limited to education, patient age specific behaviors, and dealing with the difficult patient and families.
- Adheres to agency infection control, management of hospice house hazardous waste and linen/laundry services.
- Participates in safety drills, including education, reporting, and practice implementation specific to job position.
QUALIFICATIONS
- Valid Florida RN license.
- Minimum of one year clinical experience.
- Current CPR (BLS) certification.

Description: Responsible for the planning, buying, preparation and serving of meals at the Hospice House.
Hours - Mon - Fri 8:00 a.m. - 4:30 p.m.
JOB DUTIES/KNOWLEDGE - Plans meals with high quality nutritional content for patients in collaboration with the nursing staff to ensure food content according to dietary medical orders.
- Plan for the availability of three meals plus snacks every day with the ability to have light meals and snacks at times requested by patients, ensuring as much patient choice as possible. Menu rotation.
- Conduct food inventory on a regular basis.
- Buy necessary food as economically as possible.
- Ensure safe, efficient food preparation and storage according to public health regulations and hospice house policy.
- Ensure food served from kitchen to patient room stays at proper temperatures according to regulations.
- Ensure food served to patients in isolation follows policy and procedures for isolation.
- Ensure trays, dishware and cooking utensils are sterilized according to regulations.
- Ensure kitchen equipment is in proper working order.
- Provide ongoing teaching, formal in-services, and support to food service volunteers and hospice house staff.
- Other duties as deemed necessary.
Qualifications: Must have a minimum of one year food planning experience. Experience in health care setting and/or nutritionist preferred. Must have a valid Florida drivers license and reliable transportation.

Cornerstone Hospice is seeking a compassionate Licensed Practical Nurse (LPN) to work as part of the Interdisciplinary Team and perform selected nursing care in accordance with plan of care, under the direction of the Registered Nurse and Hospice House Manager. This position provides care to patients is based at Mike Conley Hospice House in Clermont.
Schedule: 12 hour shifts, as needed. Must be able to work at least 4 shifts per month, work anywhere in the territory, if needed, and work 2-3 Holidays/year.
JOB DUTIES/KNOWLEDGE
Provides skilled palliative nursing care to patients in Hospice House as directed by the registered nurse, IDT plan of care, and physician’s orders in accordance with agency policies.
Promotes and maintains patient quality of life under the guidance of the registered nurse who is assigned to the same case.
Assists patients in learning appropriate self-care techniques. Helps to achieve and maintain continuity of patient care by assisting in planning and exchanging information with other health professionals.
Prepares clinical visit notes and follows client care plans as developed by the registered nurse and according to agency policy.
Assists the physician and registered nurse in performing specialized procedures for which competency has been established.
Prepares equipment and materials for treatments observing aseptic technique as required.
Adheres to agency infection control, management of hospice house hazardous waste and linen/laundry services.
Keeps abreast of changes and trends affecting nursing care and licensed nurse functions.
Attends client care change-of-shift conferences, meets and communicates regularly with the registered nurse.
Communicates current health status and needs of patients to appropriate health professionals. In collaboration with the registered nurse, supervises, teaches, and observes the nursing assistant(s).
QUALIFICATIONS
Valid Florida LPN license.
Minimum of three years’ clinical experience.
30-hour IV certification required; 4-hour central line certification required.
Demonstrated knowledge and skills necessary to provide care to and communicate with primarily the geriatric population, and to a lesser degree the pediatric and adult.
Demonstrated knowledge of the principles of growth and development over the life span.
Able to assess data reflecting the client’s status and interpret the appropriate information needed to identify each client’s requirements relative to their age-specific needs.

5. Observes, reports and documents change in patient status or expectations of service promptly to a supervisor or primary R.N.
6. Understands basic elements of body functioning and reports changes in patient body functions as indicated.
7. Recognizes emergency situations and implements appropriate emergency procedures when indicated.
8. Demonstrates utilization of good basic infection control procedures and follows agency procedures, especially with regard to infection control, handling of hazardous wastes, and safety measures in all instances.

Cornerstone Hospice is looking to hire a Full Time CNA to work at the Mike Conley Hospice House. The Hospice House CNA provides support services, under the supervision of the appropriate professional staff, that assist the patient and family in the achievement of physical and emotional comfort.
POSITION- 7pm-7am 3/12’s with rotating weekends. (some Holidays; Hurricane Coverage)
JOB DUTIES/KNOWLEDGE
1. Provides personal care services, such as: reading and recording temperature, pulse and respiration maintaining a clean, safe and healthy environment bed, sponge, tub or shower bathing shampooing in sink, tub or shower caring for nails and skin oral hygiene assisting with toileting and elimination.
2. Applies safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care such as ambulation of patients, transferring patients, assisting with the normal range of motion and positioning and doing household chores.
3. Prepares and provides adequate nutrition and fluid intake: provides medically prescribed diets under supervision.
4. Performs homemaking and other environmental services that facilitate receiving sanitary and safe care at home.
5. Observes, reports and documents change in patient status or expectations of service promptly to a supervisor or primary R.N.
6. Understands basic elements of body functioning and reports changes in patient body functions as indicated.
7. Recognizes emergency situations and implements appropriate emergency procedures when indicated.
8. Demonstrates utilization of good basic infection control procedures and follows agency procedures, especially with regard to infection control, handling of hazardous wastes, and safety measures in all instances.
9. Accurately documents care provided and submits visit records and mileage logs according to team policies.
10. Attends in-service programs according to agency policy. Attends patient care conferences as directed.
11. Renders services in strict accordance with the written plan of care.
QUALIFICATIONS
1. Certified by the state as a home health aide.
2. Successful completion of a nurse aide course that meets federal/state requirements.
3. Successful completion of the written competency exam and the skills demonstration evaluation required for home health aide certification by the agency.
4. Takes oral and written instruction well.
5. Works in a team responsibly and independently without direct supervision.
6. Records observations and activities
7. Good communication skills.
8. Successful completion of at least 6 months of CNA/HHA experience
9. Valid Florida drivers license and reliable transportation.

SHIFT - 8AM- 4:30 PM
JOB DUTIES/KNOWLEDGE
- Provides direct service to patients and families as needed in a variety of settings.
- Works closely with Bereavement Services Manager in formulating policies and procedures for coordination and delivery of bereavement services.
- Collaborates with Bereavement Services Manager in development of a process to evaluate patient/family outcomes in the bereavement services area.
- Coordinates all bereavement components.
- Identifies patient/family issues related to family bereavement care.
- Conducts individual and group support for families. Implements and documents bereavement services.
- Provides and documents a comprehensive support program for the bereaved according to policy which reflects respect for personal history and diversity of client.
- Continually assesses bereavement needs of patient/family.
- Ensures preparation and implementation of a written bereavement care plan for each Hospice deceased patient/family unit on Hospice premises.
- Ensures a mechanism exists to address the need for therapeutic intervention, review and referral to manager.
- Provides community programs to schools, churches, hospitals, clinics, industry, and business services regarding bereavement and grief.
- Assists in memorial services for Hospice bereaved.
- Assists bereavement support volunteers in all components of their client & bereavement activities.
- Maintains an established method of providing information in a consistent manner to staff on bereavement activities.
- Works with qualified staff social workers in the development and implementation of caregiver support groups.
- Develops and implements support groups open to the public.
- Provides individual bereavement counseling to local residents not affiliated with Hospice.
QUALIFICATIONS
- Master’s degree from a school of social work accredited by the Council on Social Work Education or a master’s degree in a related field required.
- Minimum one year of experience in social work, or a related field, preferably in a homecare agency, hospital, outpatient clinic, or mental health program in which bereavement counseling and support group facilitation was primary function.
- Must possess a Florida Licensed Clinical Social Worker (LCSW) credential in good standing or similar credential (LMHC, LMFT) required.
- Knowledge and Abilities:
- Demonstrated knowledge skills and commitment to the Hospice philosophy of care and the Hospice team concept.
- Able to assess data reflecting the patients’ status and interpret the appropriate information needed identify each patient’s requirements relative to their needs.
- Valid Florida driver’s license and required auto liability insurance.
