Cornerstone Hospice is hiring a Full Time Hospice RN Case Manager to serve Patients and Families in homes and facilities in Polk county area. The RN Case Manager plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families. This position serves areas within Polk county, FL. Must be willing to travel areas within Polk county, FL as needed/if needed.

JOB DUTIES
Provides services requiring substantial and specialized nursing skills, in accordance with the plan of care signed by the physician. May make the initial evaluation visit to the patient. Demonstrates competency in all skills required for the agency, including, but not limited to, pain and symptom control, wound care, infusion therapy, infusion therapy device care, and aseptic technique.
Completes an initial, comprehensive and ongoing assessments of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(s).
Evaluates, and regularly re-evaluates, the nursing needs of the patient. Initiates, develops, implements and makes necessary revisions to the patient’s plan of care in collaboration with IDT. Assesses the patient’s continual care needs. Addresses all problems in the plan of care or documents rationale for not doing so.
Uses health assessment data to determine nursing diagnosis. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
Initiates diagnostic and palliative nursing procedures as appropriate to the patient’s care and safety. Makes referrals to other disciplines as indicated by the patient’s needs or documents rationale for not doing so.
Observes signs and symptoms and reports to physician and/or other appropriate health professional as often as needed, or upon changes in the patient’s condition.
Teaches, supervises and counsels the patient and family regarding home nursing procedures and other care needs as appropriate to the patient’s condition. Utilizes agency educational material as appropriate.
Coordinates the total plan of care and maintains continuity of patient care by collaborating with other health professionals assigned to same patient. Attends weekly IDT meetings. Initiates patient care conferences for complex and/or multi-disciplinary patients whenever needed.
Supervises and teaches. Assesses the patient’s needs for unskilled care at least every two weeks and revises the plan of care as appropriate. Communicates the plan of care changes to the aide and scheduler as often as necessary.
Initiates & discontinues the appropriate level of care as necessary. Monitors and supervises Supportive Care staff; ensuring that care aligns with the plan of care and that services and documentation meet guidelines.
Develops, prepares and maintains individualized patient care visit records that reflect the patient’s plan of care with accuracy, timeliness, and according to policies. Submits accurate documentation within 24 hours of visit.
Follows all policies and procedures and meets all HR and Educational requirements.
Participates and adheres to the quality improvement and compliance programs.
QUALIFICATIONS
1. Valid Florida RN license.
2. Minimum one year of clinical experience.
3. CHPN preferred at the time of hire. If not eligible at the time of hire, the candidate, if selected, will be required to sit for the certification once eligible. The organization will assist the employee with preparation for the exam and covers all expenses related to the exam.

The Clinical Liaison Float (Entry Level Field Marketing/Sales Rep) is responsible for all sales and marketing activities designated to increase referrals/admissions and enhance relationships with referral sources in Polk county, FL. RN Clinician - Hospice

Cornerstone Hospice is seeking a professional Registered Nurse Clinician to oversee all activities designated to enhance and coordinate relationships with hospital referral sources and assist in timely admissions in the hospital environment. The Clinician is responsible for the case management of newly admitted GIP patients and hospitalized team patients. This includes assuring diagnostic tests, treatments, and symptom management is related to the patient's hospice diagnosis. The Clinician further facilitates timely discharge to the appropriate level of care.
SCHEDULE: Friday, Saturday & Sunday; 3/12 hour shifts ( 36 hours paid for 40).
QUALIFICATIONS
Florida R.N. license required
Clinical experience working in a hospital environment; hospice experience a plus!

JOB DUTIES/KNOWLEDGE:
Performs the social services section of the assessment process; including, but not limited to, completing the psycho-social assessment, educating the patient and family about the Hospice benefit, and gathering financial information.
Develops the plan of care with the interdisciplinary team, the patient, and the family to deal with personal, financial, and environmental difficulties experienced by the patient.
Provides social work services in accordance with the patient’s plan of care.
Assists the Interdisciplinary Team to understand the significance of social, emotional, and financial factors related to the patient’s care.
Assesses and reassesses social, emotional, and financial factors in order to help the patient and family cope with problems related to the patient’s life limiting illness.
Identifies and utilizes community and family resources to assist with the patient’s plan of care.
Develops, prepares, and maintains clinical documentation with accuracy, timeliness, and according to prescribed policies.
Contacts family after patient’s death and assesses level of coping. Makes appropriate recommendation to Bereavement Counselor for follow up.
Keeps current of hospice social services trends and knowledge. Participates in in-service programs.
Attends and participates in Interdisciplinary Team meetings. Collaborates with appropriate staff to provide social work services to patients and families.
Provides education and training for Cornerstone Hospice when requested and arranged by the Director of Social Services.
Participates in quality improvement programs.
Participates in Hospice-sponsored events.
Takes a leadership role in all issues and events relating to the psycho-social impact of life-limiting illness. Provides clinical supervision where appropriate to graduate interns and social services staff.
Provides crisis intervention for patients and their families, when and where appropriate.
Participates in on-call rotation.
QUALIFICATIONS:
Master’s degree from a school of social work accredited by the Council on Social Work Education.
Minimum one year of social work experience in a healthcare setting required.
Demonstrates knowledge, skills, and commitment to the Hospice philosophy of care and the Hospice team concept.
Possesses the ability to assess and interpret data reflecting the patient’s status, and to apply this information in a way that meets patient and family needs.

Supportive Care LPN - Nights - Marion County

Cornerstone Hospice is seeking a full-time Licensed Practical Nurse to work as part of the Interdisciplinary team supporting both Lake and Sumter county. The LPN performs selected nursing care in accordance with plan of care, under the direction of the registered nurse and nursing pool manager. The LPN must be flexible to meet the changing demands of Hospice. Regular local travel to the patient's private resident or long term care facility in Marion County. Must be willing to serve in Lake and Sumter County until Marion County is fully established.
SCHEDULE: 12 hour shifts; 8PM to 8AM ; alternating weekends (we pay mileage)
JOB SUMMARY:
- Provides skilled palliative nursing care to patients as directed by the registered nurse and IDT plan of care, in accordance with agency policies.
- Promotes and maintains patient quality of life under the guidance of the registered nurse who is assigned to the same case.
- Assists patients in learning appropriate self-care techniques.
- Helps to achieve and maintain continuity of patient care by assisting in planning and exchanging information with other health professionals.

Contract Physician - Cornerstone Hospice

The Hospice Team Contract Physician provides direct medical care as needed to patients in their homes, nursing homes, Hospice Houses and in the hospital, as well as provides physician direction and guidance to the Hospice program, staff, contractors and volunteers to ensure the maintenance of quality care for patients and families. The Contract Physician will work 10 hours/week, 40 hours/month.
- Works with Admissions staff on Certification of Terminality and provides advice regarding the Initial Care Plan for newly admitted patients.
- Advises and facilitates the implementation of hospice-appropriate management of patients’ medical and symptom needs, especially regarding selection of medications. May interact with attending physician when medical management concerns arise.
- Provides medical information and education to the team members on disease’s natural history, treatment, prognosis and palliation.
- Visits and/or examines patient to evaluate and manage specific symptom needs or ongoing problems.
- Assesses for the appropriateness of terminal diagnosis or prognosis.
- Serves as attending physician to patients/families as requested by them.
- Provides on-call coverage as needed.
- Re-certifies terminality of Hospice patients at required intervals by renewing status, writing summary, and signing appropriately.
- Communicates to Chief Medical Officer cases that require evaluation for continuing appropriateness.
- Communicates possible changes in status that may require patient to be discharged from hospice program to attending or another physician.

- Keeps abreast of hospice social services trends and knowledge.
- When requested, provides education and training for Cornerstone Hospice, participates in in service programs, and participates in Cornerstone Hospice-sponsored events.

- Oversees the provision of bereavement services in accordance with applicable laws.

QUALIFICATIONS
- Master's degree from a school of social work accredited by the Council on Social Work Education.
- Licensed Master Social Worker or Licensed Clinical Social Worker required.
- One year of experience in medical social work or a related field, or equivalent hospice internship is preferred.
- Able to demonstrate knowledge, skills, and commitment to the hospice philosophy of care and the hospice team concept.
- Able to assess data reflecting the patient's status and circumstances, and to plan interventions accordingly.
- Applicable driver license and required auto liability insurance.

Cornerstone Hospice is hiring a per diem Chaplain to support our patients and families in Orange, Osceola or Polk county. The Chaplain assists, educates, and provides support to patients and families with spiritual concerns. Bilingual (English/Spanish) a Plus!
SCHEDULE: As Needed: Monday through Friday (after hours and weekends as needed); 8:00 a.m. to 4:30 p.m.
JOB DUTIES/KNOWLEDGE:
- Provides Chaplain services in accordance with the patients plan of care.
- Assists the Inter-disciplinary team to understand the significance of spiritual factors related to the patients care.
- Assesses and reassesses spiritual factors in order to help the patient and family to cope with problems related to the patients life limiting illness.
- Recognizes and utilizes community and family spiritual supports to assist with the patients plan of care.
- Develops the plan of care with the Inter-disciplinary patient team and family to deal with spiritual difficulties experienced by the patient.
- Develops, prepares, and maintains individual patient care progress notes with accuracy, timeliness, and according to policies.
- Keeps abreast of Chaplain trends and knowledge. Participates with in-service programs. Attends and participates in Inter-disciplinary team meetings.
- Cooperates with appropriate staff to provide Chaplain services to patients and patient families.
- Provides education and training when requested and arranged by the Social Services Manager.
- Participates in quality improvement program.
- Participates in Hospice sponsored events.
- Takes a leadership role in Cornerstone Hospice issues and events relating to the spiritual impact of life limiting illness.
- Provides crisis intervention for patients and their families, when and where appropriate.
QUALIFICATIONS:
- Master of Divinity degree or similar graduate degree from a Seminary accredited by the American Association of Theological Seminaries, or the American Association of Theological schools, or similar accrediting body; and a minimum of one units of CPE or equivalent (as recognized by ACPE, APC, NACC, NAJC, CPSP, CPE-International, or ICPT).
- Two years of experience as either a pastor or chaplain in Hospice or Trauma Unit.
- Knowledge and Abilities: a) Demonstrated knowledge and commitment to the Hospice philosophy of care and the Hospice team concept. b) Able to assess data reflecting the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to their spiritual needs. c) Ability to work comfortably in a non-denominational or multi-denominational environment. d) Knowledge of multi-cultural beliefs and practices and ability to work with patients and families from diverse backgrounds, including, but not limited to, race, religion, ethnicity, or sexual orientation. e) Exceptional multi-cultural people skills; pastoral care skills; and knowledge of world religions and their end-of-life rituals.
