The Executive Director is responsible for providing strong leadership and effective regional financial performance. Ensures exceptional quality of hospice care. Operates with a high level of independence.

- Ensures agency compliance with company policies and procedure, all applicable laws and regulations, and standards as required by accreditation entities. Consistently demonstrates and demands highest levels of integrity.
- Guides and directs staff to ensure they are functioning effectively and efficiently in accordance with their respective missions. Recognizes and rewards both initiative and positive contributions of all staff.
Qualifications
1. Bachelor’s degree or equivalent experience in a healthcare-related field required; Masters preferred.
2. Five to seven years of experience in management; director or higher level preferred.
3. Hospice experience preferred.
4. CHPN required for candidates with valid Florida RN License. If the candidate does not have their CHPN, it must be obtained once the required experience period is completed.
5. Excellent communication skills, both verbal and written.
6. Ability to work collaboratively with others to achieve desired results.
7. Proficient in Excel, Power Point and Word.
8. Valid Florida driver’s license and required auto liability insurance.

Position Responsibilities:
Develops and maintains quality, professional relationships with referral sources, working professionally with physicians, case managers and staff while representing the interests of Cornerstone Hospice.
Provides information such as updates and educational materials to referral sources, answers questions, resolves problems and promotes Cornerstone Hospice services.
Follows up with manager or designee on issues raised by referral sources.
Provides Hospice information to patients/families, prepares them for admission, initiates the admission process, including principles of effective symptom management, and completes admission paperwork including verbal and written explanation of Patient Rights and Responsibilities, and any other information or consents required by regulation.
Uses established guidelines to evaluate referred patients for Hospice appropriateness, through physical assessment, review of medical documentation including History & Physical, physician notes, diagnostic testing, lab tests and other relevant information.
Provides discharge planning for patients within the hospital setting and initiates prescriptions, medical equipment, staff visits and crisis care as warranted.
Meets the needs of the newly referred patient/families, completing the admissions process as soon as possible after referral; within 24 hours or sooner.
Schedules assessment visit with patient/family, coordinating with the admissions social worker when necessary.
Gives a comprehensive explanation of hospice services and benefits addressing all components of care.
Obtains signatures on all necessary consents ensuring that patient/families are informed of their rights and responsibilities.
Performs a physical/environmental assessment, in addition to a psycho-social and spiritual assessment if an admissions social worker is not involved.
Provides primary clinical care and teaching, as needed, to stabilize the patient and facilitate the admissions process.
Documents assessments and establishes the written initial plan of care and plan of treatment.
QUALIFICATIONS
Florida R.N. license required
Clinical experience working in a hospital environment; hospice experience a plus!
Excellent oral and written skills.
Personable, strong initiative and ability to work independently.
Previous teaching/sales experience a plus.
Valid Florida driver's license and required auto liability insurance required.

Job Duties/Knowledge:
- Welcomes patients and visitors by greeting patients and visitors, in person or on the telephone. Answers or refers inquiries as appropriate.
- Responsible for researching patient member benefits, additional coverages, coverage limitations, and general coverage allowances to obtain the necessary authorizations and referrals for patient services.
- Verifies member eligibility and benefits via the health plan provider online portals or health plan provider eligibility services via fax or phone and enters the information into the EMR system accurately for authorization tracking.
- Enter and maintain accurate insurance and demographic data into patient chart
- Communicates with patients to effectively educate them on any limitations or additional services that may require contact with their insurance carrier to ensure coverage.
- Optimizes patients' satisfaction and provider time by scheduling appointments in person or by telephone.
- Keeps patient appointments on schedule by notifying provider of patient's arrival during check-in; reviews service delivery compared to schedule; reminds provider of service delays.
- Comforts patients by anticipating patients' concerns; answering patients' questions; maintaining the reception area.
- Facilitates communication between physicians and patients.
- Assists with calling in refills of medications and administering COVID-19 screening to guests.
- Ensures availability of treatment information by filing and retrieving patient records.
- Maintains patient accounts by obtaining, recording, and updating personal and financial information.
- Obtains revenue by recording and updating financial information; recording and collecting patient charges; controlling credit extended to patients; filing, collecting, and expediting third-party claims.
- Maintains business office inventory and equipment by checking stock to determine inventory level; anticipating needed supplies; placing and expediting orders for supplies; verifying receipt of supplies; scheduling equipment service and repairs.
- Protects patients' rights by maintaining confidentiality of personal and financial information.
- Maintains operations by following policies and procedures; reporting needed changes.
- Other duties as business needs determine.
Qualifications:
- High school diploma or G.E.D. required. Minimum of two years of general office experience required, with experience in a health care setting preferred.
- Communication skills to converse clearly over the telephone and in person
- Organization and time management skills to manage a variety of tasks effectively
- Attention to detail to schedule patients correctly and communicate scheduling difficulties with providers
- Technological skills, such as using Microsoft Outlook, Word, Excel and EMR programs to track data
- Interpersonal skills to interact positively with patients who may be upset or stressed

Cornerstone Hospice is looking to hire a Full-Time Registered Nurse Team Manager for our Orlando, FL location. The RN Team Manager manages all aspects (i.e. staffing, financial performance and quality) of the care provided by their respective IDT.
Schedule: M-F 8am-5pm With rotating on call
Job Duties
- Supervises daily activities of the IDT; ensuring that Patient care delivered is of the highest quality and is provided in the most efficient and cost-effective manner.
- Works with the ED to ensure availability of qualified team staff within Company guidelines. Assists in recruiting, hiring and orienting all team staff.
- Follows up and resolves internal and external concerns, incidents and suggestions. Develops processes to track and resolve recurring systems issues.
- Ensures that evaluations & competencies are completed in a timely manner and that the evaluation serves as an accurate, constructive management tool. Provides supervisory visits and coaching on a regularly scheduled basis.
- Creates scheduled reporting of Team performance. Uses available reports (i.e. KPI reporting, CAHPS reporting, departmental PPD summaries, Observation quality forms, budget statements) to monitor and manage team performance. This includes but is not limited to: documentation (quality/timeliness), staff productivity and utilization, and overall budget accountability.
- Provides education to new staff and assists all staff with needed education in collaboration with the ED and the education department. Accountable to ensure full time staff completes all mandatory training and testing within deadline.
- Performs Case Manager duties as a backup when needed.
Qualifications
- R.N. licensed in the State of Florida required, B.S.N. preferred.
- If not already certified, the Manager will be expected to obtain their CHPN within 12 months of hire date.
- Five years nursing experience required, Hospice preferred.
- Management experience strongly preferred.
- Excellent oral and written skills.
- Personable, strong initiative and ability to work independently.

JOB DUTIES/KNOWLEDGE:

Uses health assessment data to determine nursing diagnosis. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
Initiates diagnostic and palliative nursing procedures as appropriate to the patient’s care and safety. Makes referrals to other disciplines as indicated by the patient’s needs or documents rationale for not doing so.
Observes signs and symptoms and reports to physician and/or other appropriate health professional as often as needed, or upon changes in the patient’s condition.
Teaches, supervises and counsels the patient and family regarding home nursing procedures and other care needs as appropriate to the patient’s condition. Utilizes agency educational material as appropriate.
Coordinates the total plan of care and maintains continuity of patient care by collaborating with other health professionals assigned to same patient. Attends weekly IDT meetings. Initiates patient care conferences for complex and/or multi-disciplinary patients whenever needed.
Supervises and teaches. Assesses the patient’s needs for unskilled care at least every two weeks and revises the plan of care as appropriate. Communicates the plan of care changes to the aide and scheduler as often as necessary.
Initiates & discontinues the appropriate level of care as necessary. Monitors and supervises Supportive Care staff; ensuring that care aligns with the plan of care and that services and documentation meet guidelines.
Develops, prepares and maintains individualized patient care visit records that reflect the patient’s plan of care with accuracy, timeliness, and according to policies. Submits accurate documentation within 24 hours of visit.
Follows all policies and procedures and meets all HR and Educational requirements.
Participates and adheres to the quality improvement and compliance programs.

JOB DUTIES/KNOWLEDGE
- Takes a leadership role at Hospice for all issues relating to children’s bereavement.

Qualifications

JOB DUTIES/KNOWLEDGE:

- Communicates and implements systems to meet census and admission goals, while motivating staff to achieve these goals.
- Follows up and resolves internal complaints and suggestions. Develops processes to improve recurring systems issues.
- Ensures adequate staffing so patients can be admitted the day of referral to within 24 hours, including after hours and weekends, unless patient or caregiver refuses or delays the admission after the referral or unless there are documented appropriate limitations.
- Establishes regular department staff meetings with staff groups in the department on no less than quarterly schedule.
- Establishes schedule and meets individually with all assigned staff no less than twice a year for the purpose of coaching and communicating performance status including one to two times per year field supervision.
- Works collaboratively with other clinical managers and the leadership team to accomplish the strategic plans of the organization.

Notifies ordering physician and hospital case manager staff that patient is hospitalized for management of symptoms related to Hospice diagnosis that patient care is being covered under their election of the hospice benefit. Seeks opportunities to expand Hospice utilization within hospitals in critical care units, emergency departments, outpatient services and other departments and settings as may arise.
Provides creative ideas and marketing strategies for hospitals in collaboration with the Clinical Liaison assigned to the hospital. Seeks opportunities to expand Hospice services into hospital areas where there is patient need. Assists Clinical Liaisons in providing in-service programs, luncheons, etc. in hospital environment.
Performs clinician duties in hospitals throughout Cornerstone Hospice service areas as assigned by the Manager or Lead Clinician.

- Occasional after-hours administrative meetings and fund-raising events.

Qualifications
- BS or BA, preferably with coursework in business, journalism, communication, marketing/public relations.
- A minimum of 5 to 10 years (or related experience) of philanthropic and managerial work experience is required.
- Demonstrated excellence in written and oral communication skills.
- Individual must be a self-starter with the ability to guide, support, and motivate Foundation members in a positive manner, while managing multiple tasks and priorities simultaneously.
- Prefer individual with a knowledge of the region/local community.
- Knowledge and understanding of healthcare preferred but not essential.
- Knowledge of Donor Perfect donor management software helpful.
Degree of Travel
Requires extensive travel within several-county region and requires a personal means of transportation.
