Job Duties/Knowledge
- Answers phone professionally and promptly. Screens, directs and routes phone calls, relaying messages to appropriate Cornerstone staff.
- Utilizes current technology to communicate with staff.
- Maintains forms area, assuring adequate supply for team members and monitors office supplies.
- Reports daily census acuities each morning. Prepares letters, documents and memos.
- Enters all status changes into the electronic medical record.
- Reads and responds to emails in a professional and prompt manner.
- Reviews home health aide schedule daily, updating as necessary and notifies team staff and designated support staff of patient home health aide schedule changes.
- Upon discharge, revocation or death of patient, completes conclusion of Service checklist tasks.
- Assists in the creation of the IDT agenda, including lists of re-certifications and face-to-face visit dates.
- Organizes, prepares and updates IDT materials and participates in IDT as needed.
- Distributes inter-office mail to team.
- Copies, collates and distributes paperwork and documentation intra- and inter-departmentally.

JOB DUTIES
Provides services requiring substantial and specialized nursing skills, in accordance with the plan of care signed by the physician. May make the initial evaluation visit to the patient. Demonstrates competency in all skills required for the agency, including, but not limited to, pain and symptom control, wound care, infusion therapy, infusion therapy device care, and aseptic technique.
Completes an initial, comprehensive and ongoing assessments of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(s).
Evaluates, and regularly re-evaluates, the nursing needs of the patient. Initiates, develops, implements and makes necessary revisions to the patient’s plan of care in collaboration with IDT. Assesses the patient’s continual care needs. Addresses all problems in the plan of care or documents rationale for not doing so.
Uses health assessment data to determine nursing diagnosis. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
Initiates diagnostic and palliative nursing procedures as appropriate to the patient’s care and safety. Makes referrals to other disciplines as indicated by the patient’s needs or documents rationale for not doing so.
Observes signs and symptoms and reports to physician and/or other appropriate health professional as often as needed, or upon changes in the patient’s condition.
Teaches, supervises and counsels the patient and family regarding home nursing procedures and other care needs as appropriate to the patient’s condition. Utilizes agency educational material as appropriate.
Coordinates the total plan of care and maintains continuity of patient care by collaborating with other health professionals assigned to same patient. Attends weekly IDT meetings. Initiates patient care conferences for complex and/or multi-disciplinary patients whenever needed.
Supervises and teaches. Assesses the patient’s needs for unskilled care at least every two weeks and revises the plan of care as appropriate. Communicates the plan of care changes to the aide and scheduler as often as necessary.
Initiates & discontinues the appropriate level of care as necessary. Monitors and supervises Supportive Care staff; ensuring that care aligns with the plan of care and that services and documentation meet guidelines.
Develops, prepares and maintains individualized patient care visit records that reflect the patient’s plan of care with accuracy, timeliness, and according to policies. Submits accurate documentation within 24 hours of visit.
Follows all policies and procedures and meets all HR and Educational requirements.
Participates and adheres to the quality improvement and compliance programs.
QUALIFICATIONS
1. Valid Florida RN license.
2. Minimum one year of clinical experience.
3. CHPN preferred at the time of hire. If not eligible at the time of hire, the candidate, if selected, will be required to sit for the certification once eligible. The organization will assist the employee with preparation for the exam and covers all expenses related to the exam.

Cornerstone Hospice is looking to hire a Full-Time Registered Nurse Team Manager for the Villages location. The RN Team Manager manages all aspects (i.e. staffing, financial performance and quality) of the care provided by their respective IDT.
Schedule: M-F 8am-5pm With rotating on call
Job Duties
- Supervises daily activities of the IDT; ensuring that Patient care delivered is of the highest quality and is provided in the most efficient and cost-effective manner.
- Works with the ED to ensure availability of qualified team staff within Company guidelines. Assists in recruiting, hiring and orienting all team staff.
- Follows up and resolves internal and external concerns, incidents and suggestions. Develops processes to track and resolve recurring systems issues.
- Ensures that evaluations & competencies are completed in a timely manner and that the evaluation serves as an accurate, constructive management tool. Provides supervisory visits and coaching on a regularly scheduled basis.
- Creates scheduled reporting of Team performance. Uses available reports (i.e. KPI reporting, CAHPS reporting, departmental PPD summaries, Observation quality forms, budget statements) to monitor and manage team performance. This includes but is not limited to: documentation (quality/timeliness), staff productivity and utilization, and overall budget accountability.
- Provides education to new staff and assists all staff with needed education in collaboration with the ED and the education department. Accountable to ensure full time staff completes all mandatory training and testing within deadline.
- Performs Case Manager duties as a backup when needed.
Qualifications
- R.N. licensed in the State of Georgia required, B.S.N. preferred.
- If not already certified, the Manager will be expected to obtain their CHPN within 12 months of hire date.
- Five years nursing experience required, Hospice preferred.
- Management experience strongly preferred.
- Excellent oral and written skills.
- Personable, strong initiative and ability to work independently.
- Valid GA driver’s license, reliable transportation and agency-required liability insurance.

JOB DUTIES/KNOWLEDGE
Vacuums, dusts, empties trash and thoroughly cleans all assigned areas in a manner and frequency as required.
Cleans and disinfects patient care equipment such as bed, over-bed tables, beside commode at room change in accordance with OSHA requirements.
Responds promptly and cheerfully to requests for emergency clean-up service.
Promptly reports to supervisor any needed repairs, replacement or maintenance issues, including hazardous conditions.
Other duties as assigned by supervisor.
QUALIFICATIONS
High school diploma required.
Minimum of one year housekeeping experience required.

Hospice Chaplain: Assists, educates, and provides support to patients and families with spiritual concerns.

Provides Chaplain services in accordance with the patients plan of care.
Assists the Inter-disciplinary team to understand the significance of spiritual factors related to the patients care.
Assesses and reassesses spiritual factors in order to help the patient and family to cope with problems related to the patients life limiting illness.
Recognizes and utilizes community and family spiritual supports to assist with the patients plan of care.
Develops the plan of care with the Inter-disciplinary patient team and family to deal<BR>\with spiritual difficulties experienced by the patient.
Develops, prepares, and maintains individual patient care progress notes with accuracy, timeliness, and according to policies.
Keeps abreast of Chaplain trends and knowledge. Participates with in-service programs. Attends and participates in Inter-disciplinary team meetings. Cooperates with appropriate staff to provide Chaplain services to patients and patient families.<BR>9. Provides education and training for Hospice of Lake & Sumter, when requested and arranged by the Social Services Manager.
Participates in quality improvement program.
Participates in Hospice sponsored events.
Takes a leadership role in Cornerstone Hospice issues and events relating to the spiritual impact of life limiting illness.
Provides crisis intervention for patients and their families, when and where appropriate.
Qualifications:
1. Master of Divinity degree or similar graduate degree from a Seminary accredited by the American Association of Theological Seminaries, or the American Association of Theological schools, or similar accrediting body; and a minimum of one unit of CPE or equivalent (as recognized by ACPE, APC, NACC, NAJC, CPSP, CPE-International, or ICPT).
2. Two years of experience as either a pastor or chaplain in a Hospice House and/or Trauma Unit.
3. Knowledge and Abilities:
a) Demonstrated knowledge and commitment to the Hospice philosophy of care and the Hospice team concept
b) Able to assess data reflecting the patients status and interpret the appropriate information needed to identify each patients requirements relative to their spiritual needs.
c) Ability to work comfortably in a non-denominational or multi-denominational environment.
d) Knowledge of multi-cultural beliefs and practices
e) Exceptional multi-cultural people skills; pastoral care skills; and knowledge of world religions and their end-of-life rituals.

Assists the physician and registered nurse in performing specialized procedures for which competency has been established.
Prepares equipment and materials for treatments observing aseptic technique as required.

Keeps abreast of changes and trends affecting nursing care and licensed nurse functions.
Attends client care change-of-shift conferences, meets and communicates regularly with the registered nurse.

QUALIFICATIONS
Valid Florida LPN license.
Minimum of three years’ clinical experience.
30-hour IV certification required; 4-hour central line certification required.

- Provides medical information and education to the team members on disease’s natural history, treatment, prognosis and palliation.
- Visits and/or examines patient to evaluate and manage specific symptom needs or ongoing problems.
- Assesses for the appropriateness of terminal diagnosis or prognosis.
- Serves as attending physician to patients/families as requested by them.
- Provides on-call coverage as needed.
- Re-certifies terminality of Hospice patients at required intervals by renewing status, writing summary, and signing appropriately.
- Communicates to Chief Medical Officer cases that require evaluation for continuing appropriateness.
- Communicates possible changes in status that may require patient to be discharged from hospice program to attending or another physician.

- Attends IDT meetings and fully supports the interdisciplinary team, utilizing members of each discipline in choosing interventions for the benefit of each patient and recognizing successes and strengths in fellow team members.
- Provides Face-to-Face (F2F) visits as required by Medicare in a timely manner in conjunction with APRN at Medicare-determined intervals.
Qualifications:
1. Licensed as a physician in the state of Florida.

specialty, or other approved medical specialty, having maintained requirements for

Gives a comprehensive explanation of hospice services and benefits addressing all components of care.
Obtains signatures on all necessary consents ensuring that patient/families are informed of their rights and responsibilities.
Performs a physical/environmental assessment, in addition to a psycho-social and spiritual assessment if an admissions social worker is not involved.
Provides primary clinical care and teaching, as needed, to stabilize the patient and facilitate the admissions process.
