In addition, the On-Call Registered Nurse (Runner) is responsible for identifying immediate patient or family needs and providing supportive care and appropriate services in accordance with the attending physician’s orders, the interdisciplinary team plan of care, and the policies and procedures of Cornerstone Hospice.

JOB DUTIES/KNOWLEDGE
- Assumes responsibility upon dispatch for after –hours’ requests for on-site support. Evaluates and determines the need for clinical and other services including assessments, symptom management and care plan updates, documenting in Allscripts (EMR) and coordinating these with appropriate Cornerstone Hospice staff members.
- Obtains data on current or potential physical, psychological, social and spiritual factors that may impact patient and family’s needs and initiate intervention by other members of the interdisciplinary team, as appropriate.
- Initiates SBAR communication with the attending physician, other interdisciplinary staff members and other agencies to coordinate optimal care.
- Effectively utilizes resources to assist in meeting the patient’s needs.
- Performs therapeutic, preventative and rehabilitative nursing procedures as directed by the attending physician.
- Performs clinical pronouncement of death and provide support to the family.
- Performs initial assessment of individuals referred for care, determine eligibility under Cornerstone Hospice criteria. Performs admission of the patient as appropriate.
- Maintains an accurate clinical record through the timely preparation and documentation of care plans and clinical and progress notes utilizing EMR.
- Maintains regular communication with members of the interdisciplinary team, alerting attending physician and assigned Case Manager to any changes in the patient’s condition or needs as well as potential problems or issues.
- Provides effective teaching to family, caregivers and interdisciplinary team members to ensure continuity of care for the patient.
- Develops and maintains familiarity with Federal, State, and Cornerstone policies and procedures.
- Accepts responsibility for personal professional growth/development through participation in continuing education programs, agency in-services and community events.
- Adheres to work schedule and maintains good punctuality and attendance record
- Other duties as assigned.
QUALIFICATIONS:
- Valid Florida RN license.
- Minimum three years of clinical experience or Hospice preceptorship program.
- Demonstrated knowledge and skills necessary to provide care to and communicate with primarily the geriatric population, and to a lesser degree, the pediatric and adult populations.
- Demonstrated knowledge of the principles of growth and development over the life span
- Able to assess data reflecting the client’s status and interpret the appropriate information needed to identify each client’s requirements relative to their age-specific needs.

Community Liaison - Field Marketing & Outreach

The Community Liaison provides and assists with community relations activities that promote the agency’s goals, strengthen credibility and establish and maintain goodwill and mutual understanding between the organization and the community primarily in The Villages and surrounding areas. Provides information and community education regarding hospice philosophy and services; to increase public knowledge of services and to raise awareness of Cornerstone Hospice in the community. Territory: Calling on all home health partners, health departments, and back up for clinical liaisons/Marketing reps as needed.

Job Duties:
- Working with the Community Relations Manager, and support of the VP of Sales and Marketing, serves as spokesperson to the agency’s various public in assigned area to build and maintain relationships.
- Establishes and maintains relationships with government representatives at the local, county and state levels and represents the agency at designated events.
- Assures Cornerstone Hospice representation at appropriate chambers of commerce, civic, and community meetings in areas served by Cornerstone Hospice.
- Represents Cornerstone Hospice at local civic and community events, groups and committees as relates to agency objectives.
- In conjunction with the Community Relations Manager, and support of the VP of Sales and Marketing, coordinates, schedules and organizes other community outreach events and activities that further the goals and objectives of the marketing and strategic plans.
- With the Community Relations Manager, and support of the VP of Sales and Marketing, coordinates and manages Cornerstone Hospice local speakers’ bureau and church ambassador’s program.
- Seeks public speaking opportunities; serves as a public speaker as needed.
- Gathers information for agency web site and assists Community Relations Manager in keeping site current.
- Conducts local market research that includes population and healthcare trends, public needs, and knowledge and perception of the agency and its services. Includes such activities as conducting focus groups, coordinating surveys and researching reports, newspapers, literature and other information media.
- Implements public relations strategies that will influence public opinion and promote Hospice concepts and programs.
- Assists with public relations aspect of possible crisis situations.
- Works with members of the Marketing team to develop and implement initiatives that enhance the image and awareness of Cornerstone Hospice in the community and assure that all who need and desire Hospice care receive services.
- Makes education for CEU’s available and presents to the public.
- Keeps the Community Relations Manager and VP of Sales and Marketing informed of planned activities and outcomes to facilitate coordinated Marketing departmental and Community Relations division initiatives.
- Coordinates own role and duties; assists activities of Community Relations and Public Information, Clinical Liaisons, Minority Outreach, and Interfaith staff throughout assigned service area.
- Achieves established census goals in assigned territory (3E, 6B, or 7B), through community relations events, education, community involvement, etc.
Qualifications:
- High School Diploma.
- A minimum of three years of experience in public relations or equivalent preferred. Sales & Marketing experience preferred.
- Effective verbal and written communication skills in English, appropriate to situation and objectives.
- Knowledge of principles and methods for promoting services.

- Provides services requiring substantial and specialized nursing skills, in accordance with the plan of care signed by the physician. May make the initial evaluation visit to the patient.

The Business Intelligence Developer/Programmer is responsible for supporting and delivering BI functionality to support the reporting/analytics needs of Cornerstone. - Work closely with the user community understanding their data reporting requirements, building data marts and data warehouses to service the data analytics and reporting needs of this dynamic business.

- Ensure that all projects are delivered on-time, within the scope of the project, and within budget in coordination with the project management team.
- Facilitate or lead in the definition of a data dictionary and metric documentation.
- Perform data validation and data profiling on source systems.
- Become a subject matter expert with chosen business intelligence tools and development standards and best practices related to reports and all related objects (Dashboards, Attributes, filters, metrics, facts, prompts, templates, consolidation, and custom groups).
- Integrate with, support, and share responsibilities with internal web, application, and system development efforts.
- Troubleshoot and support all SQL Server reporting and BI related production processing.
- Will participate in the implementation and/or testing phases of integration and metric development.
- Ensure proper change management controls are implemented for your sphere of influence.

Qualifications
- Four (4) year Bachelor of Science degree or equivalent experience required, preferably in computer science or related field.
- Demonstrates at least 4 years of successful IT development experience.
- Has at least 4 years of experience in information technology roles.
- Has at least 2 years of experience developing/using SQL with moderate to advanced skills.
- Has at least 2 years of experience using data extraction tools.
- Strong analytical and problem-solving skills with the ability to focus on results, quality, and satisfaction.
- Certification in SQL or Business Intelligence preferred.
- Knowledge of Data Warehouse, ETL concept and tools.
- Experience in SSMS, SSAS, SSRS, SSIS, TSQL, DAX, and Python preferred.
- Experience with at least one major Business Intelligence toolset.
- Solid technical background with understanding of business applications, IT applications, and IT infrastructure.
- Healthcare experience a plus.

Follows up and resolves internal and external concerns, incidents and suggestions. Develops processes to improve recurring system issues.
Ensures that evaluations & competencies are completed in a timely manner and that the evaluation serves as an accurate, constructive management tool. Provides supervisory visits on a regularly scheduled basis.
Uses available reports (i.e. Amplicare, monthly dashboards and departmental statements) to monitor & manage documentation, quality, utilization, quality, compliance and financial performance.
Provides education to new staff and assists all staff with needed education in collaboration with the Executive Director and the education department.
Performs Case Manager duties as a backup when needed.
Ensures compliance with organizational policies and procedures and with relevant Laws and Regulations.

Job Summary:
- Identify, research, and cultivate prospects in assigned service area who have the ability to contribute to the Cornerstone Hospice mission through monetary donation and/or community leadership i.e. event creation, advisory board service and other special events and projects.
- Develop, implement, communicate with and educate community groups and organizations of the mission of Cornerstone Hospice as it relates to the Foundation’s initiatives.

- Observes signs and symptoms and reports to physician and/or other appropriate health professional as often as needed, or upon changes in the patient’s condition.

Position Responsibilities:
Develops and maintains quality, professional relationships with referral sources, working professionally with physicians, case managers and staff while representing the interests of Cornerstone Hospice.

Follows up with manager or designee on issues raised by referral sources.
Provides Hospice information to patients/families, prepares them for admission, initiates the admission process, including principles of effective symptom management, and completes admission paperwork including verbal and written explanation of Patient Rights and Responsibilities, and any other information or consents required by regulation.
Uses established guidelines to evaluate referred patients for Hospice appropriateness, through physical assessment, review of medical documentation including History & Physical, physician notes, diagnostic testing, lab tests and other relevant information.
Provides discharge planning for patients within the hospital setting and initiates prescriptions, medical equipment, staff visits and crisis care as warranted.
Meets the needs of the newly referred patient/families, completing the admissions process as soon as possible after referral; within 24 hours or sooner.
Schedules assessment visit with patient/family, coordinating with the admissions social worker when necessary.
Gives a comprehensive explanation of hospice services and benefits addressing all components of care.
Obtains signatures on all necessary consents ensuring that patient/families are informed of their rights and responsibilities.
Performs a physical/environmental assessment, in addition to a psycho-social and spiritual assessment if an admissions social worker is not involved.
Provides primary clinical care and teaching, as needed, to stabilize the patient and facilitate the admissions process.
Documents assessments and establishes the written initial plan of care and plan of treatment.
Qualifications

JOB DUTIES/KNOWLEDGE
- Provides services requiring substantial and specialized nursing skills, in accordance with the plan of care signed by the physician. May make the initial evaluation visit to the patient. Demonstrates competency in all skills required for the agency, including, but not limited to, pain and symptom control, wound care, infusion therapy, infusion therapy device care, and aseptic technique.
- Evaluates the nursing needs of the patient. Initiates, develops, implements and makes necessary revisions to the patient’s plan of care in collaboration with IDT. Assesses the patient’s continual care needs. Addresses all problems in the plan of care or documents rationale for not doing so.
- Initiates diagnostic and palliative nursing procedures as appropriate to the patient’s care and safety. Makes referrals to other disciplines as indicated by the patient’s needs or documents rationale for not doing so.
- Observes signs and symptoms and reports to physician and/or other appropriate health professional as often as needed, or upon changes in the patient’s condition.
- Teaches, supervises and counsels the patient and family regarding home nursing procedures and other care needs as appropriate to the patient’s condition. Utilizes agency educational material as appropriate.
- Develops, prepares and maintains individualized patient care visit records that reflect the patient’s plan of care with accuracy, timeliness, and according to policies. Submits accurate documentation within 24 hours of visit.
- Responsible for following all policies and procedures of the agency regarding service delivery documentation and care coordination. During supervisory visits exhibits full compliance and verbalizes knowledge of agency policies including, but not limited to, patient education, infection control, management of hazardous wastes and age-specific behaviors for patients.
QUALIFICATIONS
- Valid Florida RN license.

JOB DUTIES/KNOWLEDGE Supervises daily activities of the IDT; ensuring that the care that is delivered is of the highest quality and provided in the most cost effective manner. Works with the Executive Director to ensure availability of qualified team staff. Assists in recruiting, hiring and orienting all team staff. Follows up and resolves internal and external concerns, incidents and suggestions. Develops processes to improve recurring system issues. Ensures that evaluations & competencies are completed in a timely manner and that the evaluation serves as an accurate, constructive management tool. Provides supervisory visits on a regularly scheduled basis. Uses available reports (i.e. Amplicare, monthly dashboards and departmental statements) to monitor & manage documentation, quality, utilization, quality, compliance and financial performance. Provides education to new staff and assists all staff with needed education in collaboration with the Executive Director and the education department. Performs Case Manager duties as a backup when needed. Ensures compliance with organizational policies and procedures and with relevant Laws and Regulations. Consistently facilitates positive interpersonal relations among staff. Ensures ongoing communication and collaboration with community partners and referral sources (i.e. physicians SNF, ALFs, and Hospitals).
QUALIFICATIONS State RN license required, B.S.N. preferred. If not already certified, the Manager will be expected to obtain their CHPN within 12 months of hire date. Five years nursing experience required, Hospice preferred. Management experience strongly preferred. Basic understanding of the interpersonal dynamics within the working relationships of an interdisciplinary health care team, and knowledge of the process of dying and bereavement. Excellent oral and written skills. Personable, strong initiative and ability to work independently. Valid state driver’s license, reliable transportation and agency-required liability insurance. Must be able to work flexible hours.

• Works as part of the HIM team to ensure timely submission and retention of records.

• Scans and indexes patient medical records into the electronic health record.

• Processes system generated, and paper physician created orders. Ensure orders are submitted to the appropriate physician for signature as required, tracking receipt of the signed documents in adherence of federal and state signing requirements.
• Monitors compliance with documentation standards; researches, investigates, evaluates and identifies opportunities for improvement. • Provides written and verbal reports to be utilized by leadership as directed by the Compliance & HIM Director.
• Assures HIPAA compliance: a) All patient records are locked at the close of work day. b) Keys to locked record area(s) are secured daily per department policy. c) Computer is locked at all times when the employee is away from the work station. d) Verifies valid written release of information has been obtained before releasing any or all patient information. e) Verifies correct fax number prior to faxing patient information. f) Provides patient information only to hospice staff members that have “need to know”. g) Does not conduct conversations regarding patient information in public areas. • Responds to the needs of the agency and the department by performing other duties as assigned.
QUALILFICATIONS • High school diploma or equivalent required.
• Minimum two years of clerical experience required; health information management or office experience preferred.
• Knowledge of medical terminology preferred.
• Ability to file alphabetically as well as numerically.
• Good computer skills in Word and Excel and ability to perform data entry tasks.

Attends and participates in IDT process. Documents nursing assessments, nurses’ notes, medications records, addressing problems in the plan of care or/either rationale for not doing so.
Follows physicians orders when initiating medical and palliative nursing procedures as appropriate to the patient’s care and safety. Makes referrals to other disciplines as indicated by the patient’s needs or documents rationale for not doing so.
QUALIFICATIONS
Required Florida RN license. Required Associates degree, Bachelor’s degree preferred.
Minimum of one year clinical experience.
Current CPR (BLS) certification.
Knowledge and Abilities:
a) Demonstrated knowledge and skills necessary to provide care to and communicate with primarily the geriatric population, and to a lesser degree the pediatric and adult populations.

Assists the physician and registered nurse in performing specialized procedures for which competency has been established.
Prepares equipment and materials for treatments observing aseptic technique as required.
Adheres to agency infection control, management of hospice house hazardous waste and linen/laundry services.
Keeps abreast of changes and trends affecting nursing care and licensed nurse functions.
Attends client care change-of-shift conferences, meets and communicates regularly with the registered nurse.
Communicates current health status and needs of patients to appropriate health professionals. In collaboration with the registered nurse, supervises, teaches, and observes the nursing assistant(s).
QUALIFICATIONS
Valid Florida LPN license.
Minimum of three years’ clinical experience.
30-hour IV certification required; 4-hour central line certification required.
Demonstrated knowledge and skills necessary to provide care to and communicate with primarily the geriatric population, and to a lesser degree the pediatric and adult.
Demonstrated knowledge of the principles of growth and development over the life span.

The Admissions Nurse is a qualified professional nurse responsible for determining eligibility and preparing patients/families for hospice care. For patients discharged from the hospital outside of regular work hours, the RN will be responsible for securing orders, initiating symptom management, completing documentation, and confirming services and treatments with the patient’s attending physician. The role of Admissions Nurse also includes tuck in for hospice patients discharged from the hospital to home or facility. Additionally, the role includes assisting the after hours On Call team with visits and calls. The position requires high level nursing assessment and communication skills.

This position will care for patients residing in private homes and LTC facilities in Polk, Hardee and/or Highlands counties.

JOB DUTIES/KNOWLEDGE
- Provides services requiring substantial and specialized nursing skills, in accordance with the plan of care signed by the physician. May make the initial evaluation visit to the patient. Demonstrates competency in all skills required for the agency, including, but not limited to, pain and symptom control, wound care, infusion therapy, infusion therapy device care, and aseptic technique.
- Evaluates the nursing needs of the patient. Initiates, develops, implements and makes necessary revisions to the patient’s plan of care in collaboration with IDT. Assesses the patient’s continual care needs. Addresses all problems in the plan of care or documents rationale for not doing so.
- Initiates diagnostic and palliative nursing procedures as appropriate to the patient’s care and safety. Makes referrals to other disciplines as indicated by the patient’s needs or documents rationale for not doing so.
- Observes signs and symptoms and reports to physician and/or other appropriate health professional as often as needed, or upon changes in the patient’s condition.
- Teaches, supervises and counsels the patient and family regarding home nursing procedures and other care needs as appropriate to the patient’s condition. Utilizes agency educational material as appropriate.
- Develops, prepares and maintains individualized patient care visit records that reflect the patient’s plan of care with accuracy, timeliness, and according to policies. Submits accurate documentation within 24 hours of visit.
- Responsible for following all policies and procedures of the agency regarding service delivery documentation and care coordination. During supervisory visits exhibits full compliance and verbalizes knowledge of agency policies including, but not limited to, patient education, infection control, management of hazardous wastes and age-specific behaviors for patients.

Qualifications

- Minimum one year of experience in social work, or a related field, preferably in a home care agency, hospital, outpatient clinic, or mental health program in which bereavement counseling and support group facilitation was primary function.
- Must possess a Florida Licensed Clinical Social Worker (LCSW) credential in good standing or similar credential (LMHC, LMFT) required.

The Corporate Clinical Educator RN, is responsible for all educational activities designated to increase referrals/admissions, including self referrals, and to enhance relationships with established and target referral sources. These activities may include but are not limited to marketing, public relations, public speaking, and education. JOB DUTIES/ KNOWLEDGE:
- Develops and maintains quality professional relationships with established referral sources.
- Establishes and builds relationships with organizations that impact all program areas. Providing regular feedback on individual patients as requested, creating opportunities for ongoing partnership and education. Provides a consistent professional presence in the community. This includes: Participation in appropriate groups and working with these groups before/after 8:30 a.m. – 5:00 p.m. business day and weekend days as required to meet admission goals.
- Develops a comprehensive marketing plan including goals, activities and timelines, in collaboration with the sales team.
- Collaborates with the Business Development Managers in developing educational Programs relevant to the audience and community’s needs. Maintains current updated educational materials, meeting the CEU requirements for the State Board of Nursing and/or other crediting organizations.
- Responsible for supporting each program area, and its Clinical Liaisons, with a minimum of 12 educational programs a month, to the professional, civic, community organizations, and general public audiences.
- Increase referral generation, while focusing on improved length of stay (LOS).
- Provides information to referral sources, such as updates, educational material and statistical analysis. Is a resource for questions pertaining to hospice, palliative care and Alexa. Provides consistent, frequent communication to referral sources in collaboration with other members of the sales team. This includes but it not limited to our organizational quality, competencies and unique offerings.
- Follows through with appropriate hospice leadership and members of the liaison team to resolves issues raised by referral sources
- Develop and implement pertinent educational programs. Including marketing and education to a variety of audiences. Represents hospice at public meetings, boards, and activities as defined in the marketing plan or as required by leadership. Consistently looks for new and creative ideas, new markets and marketing strategies to discuss with the marketing team. Is an active, enthusiastic member of the marketing team. Constantly strives to improve the image of Cornerstone Hospice in the provider community.
- Assists with total number of quarterly admissions credited to Demographic Branch.
QUALIFICATIONS:
- Valid Florida RN license required; BSN and/or master’s degree preferred.
- Three years of professional medical marketing experience and/or hospice experience preferred.
- Ability to promote hospice with potential referral sources in competitive areas.
- Outgoing with excellent communication skills, including public speaking experience.

RESPONSIBILITIES

- Assesses the patient’s continual care needs and collaborates with Hospice House Manager, RTM, or Patient Care Supervisor and team members regarding patient’s plan of care. Attends and participates in IDT process. Documents nursing assessments, nurses’ notes, medications records, addressing problems in the plan of care or/either rationale for not doing so.
- Follows physicians orders when initiating medical and palliative nursing procedures as appropriate to the patient’s care and safety. Makes referrals to other disciplines as indicated by the patient’s needs or documents rationale for not doing so.
- Observes signs and symptoms and reports to physician and/or other appropriate health professional as often as needed, or upon changes in the patient’s condition.
- Provides services requiring substantial and specialized nursing skills, in accordance with the physician orders. Demonstrates competency in all skills required for the agency, including, but not limited to, venipunctures, wound care, infusion therapy, medication administration, infusion therapy device care, and aseptic technique.
- Teaches, supervises and counsels the patient and family regarding procedures and other care needs as appropriate to the patient’s condition. Utilizes agency educational material as appropriate.
- Supervises the LPN and teaches the nursing assistant where applicable. Assesses the patient’s need for personal care, activities of daily living and revises the plan of care as appropriate. Communicates the plan of care assignments to the assistant and reviews assistants documentation of care as necessary.
- Responsible for following all policies and procedures of hospice and regulatory agencies including, but not limited to, documentation, ordering, storage and record keeping of controlled medications.
- Verbalizes and exhibits knowledge of dealing with patients and families including, but not limited to education, patient age specific behaviors, and dealing with the difficult patient and families.
- Adheres to agency infection control, management of hospice house hazardous waste and linen/laundry services.
- Participates in safety drills, including education, reporting, and practice implementation specific to job position.
QUALIFICATIONS

1. Masters’ degree from a school of social work accredited by the Council on Social Work Education (CSWE) or Masters’ degree in counseling, psychology or related field required.
2. Must possess a Florida Licensed Clinical Social Worker (LCSW) credential in good standing or similar credential (LMHC, LMFT) and have a minimum of 5years experience providing mental health/behavioral health counseling required.
3. Able to demonstrate knowledge, skills, and commitment to the mental health philosophy.
4. Able to assess data reflecting the client’s status and circumstances, and to plan interventions accordingly.
